Event Terms & Conditions:
Assumption of Risk
I understand that participation in the Bill Waris Blue Jays Basketball Camp is a physical activity that involves inherent risks, including but not limited to sprains, strains, fractures, collisions with other participants, and other injuries. I voluntarily assume all risks associated with participation in this event.
Medical Authorization
In the event of an emergency, I authorize Bill Waris Blue Jays Basketball Camp staff and coaches to provide first aid and to secure medical treatment for the participant if necessary. I understand that I am responsible for any and all medical expenses incurred.
Release of Liability
I hereby release, waive, and discharge the Bill Waris Blue Jays Basketball Camp, its directors, coaches, staff, volunteers, facility owners, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or loss sustained while participating in event activities
Code of Conduct
Participants are expected to demonstrate respect toward coaches, officials, and other players. Unsafe behavior, disrespect, or repeated violations of camp rules may result in removal from the Bill Waris Blue Jays Basketball event without refund.
Parent/Guardian Consent
I certify that I am the parent or legal guardian of the participant and that I have read and agree to all terms of this waiver for the Bill Waris Blue Jays Basketball event.
Photo & Video Release
I grant permission for the Bill Waris Blue Jays Basketball Camp to use photographs and/or video of the participant for promotional, educational, and marketing purposes, including social media and website use.


Club Parent/Guardian Financial Agreement & Parent/Player Agreement
By registering for the Bill Waris Blue Jays Basketball Club, I acknowledge that I have read, understand, and agree to the following policies and expectations.
Program Commitment
Participation in the Bill Waris Blue Jays Basketball Club is a seasonal commitment. I understand that my child is committing to the session(s) in which they are enrolled, including Winter, Spring, Summer, and Fall.Players are expected to attend practices, games, and tournaments whenever possible. If an absence is necessary, parents and players should notify the coaching staff as soon as possible.
Financial Responsibility
I agree to pay all fees associated with my child's participation in the program.
-Monthly program fee: $125, due on the 1st day of each month
-Uniform package: $85 (separate fee)
-Tournament fees: Billed separately as applicable
Failure to attend practices, games, or tournaments does not relieve me of my financial obligation for the session in which my child is enrolled.
Payment, Refund & Account Policy
All registration fees, monthly program fees, uniform fees, and tournament fees are non-refundable once paid. No refunds or credits will be issued for:
-Missed practices, games, or tournaments
-Schedule changes
-Voluntary withdrawal from the program
-Dismissal from the program
-Any other reason
-Registration fees become non-refundable once a player has been placed on a team unless otherwise approved by the Club Director.
Tournament fees already committed by the club are also non-refundable. Accounts not paid by the due date may be subject to a late fee at the discretion of the Bill Waris Blue Jays Basketball Club. Players with outstanding balances may be withheld from practices, games, tournaments, or other team activities until their account is brought current. The Club also reserves the right to suspend or remove a player from the program for failure to meet financial obligations.

Playing time is not guaranteed and is determined by several factors, including:
Practice attendance
Effort
Attitude
Coachability
Game situations
Team needs
Parent Expectations
Parents play an important role in creating a positive environment for every athlete. Parents are expected to:
Support all players on the team.
Treat coaches, officials, opponents, and spectators with respect.
Encourage good sportsmanship at all times.
Refrain from coaching players from the stands during practices or games.
Follow the 24-hour rule, waiting at least 24 hours after a game before discussing concerns with a coach unless the matter involves player safety.
Player Expectations
Players are expected to:
Display respect toward coaches, teammates, officials, opponents, and facilities.
Give their best effort at every practice, game, and tournament. Demonstrate good sportsmanship and a positive attitude and represent the Bill Waris Blue Jays Basketball Club with integrity on and off the court.
Weather & Schedule Changes
Practice, game, and tournament schedules may occasionally change due to weather, facility availability, or unforeseen circumstances. The Bill Waris Blue Jays Basketball Club will communicate any schedule changes as soon as reasonably practical.
